ÖZET
Amaç: Bu çalışma transdermal östradiol tedavisinin uzun süreli kullanımlarında güvenilirliğini araştırmak için yapıldı. Çalışmanın Yapıldığı Yer: GATA Haydarpaşa Eğitim Hastanesi, Kadın Hastalıkları ve Doğum Kliniği, İstanbul. Materyel ve Metod: Transdermal östradiol tedavisinin uzun süreli kullanımlarında güvenilirliğini araştırmak için, randomize 40 postmenopozal hasta üzerinde araştırma yapıldı. 1. grup hasta, 20 kişi, 28 günlük bir siklusun 25 günü 0.05 mg transdermal östradiol alırken, 2. grup 20 kişilik hasta grubu ise aynı dozajla birlikte siklusun 13-25. günleri arasında 10 mg medroksiprogesteron asetat aldılar. Tedavi süresi ortalama olarak 1 yıl sürdü. Endometrial histolojiler ve vajinal kanama şekilleri takip edildi. Vajinal kanamalar hastalar tarafından tespit edilirken, 6. ve 12. aylarda endometrial biopsiler alındı. Bulgular: Kanama başlangıç, süre ve miktarları her iki grup için de benzer idi. Hiperplazi insidansı birinci grupta %35 iken, ikinci grupta ise %5 idi. Birinci gruptaki tüm hiperplazi vakaları daha sonra 3 aylık medroksiprogesteron asetat tedavisine alındı ve tekrar biopsi ile değerlendirildiler. Hepsinde normal endometrium tespit edildi. Sonuç: Endometrial histoloji sonuçları ile elde ettiğimiz bilgilere göre, transdermal östradiol, siklik progestinler ile kullanıldığı takdirde güvenli olmaktadır.
ABSTRACT
Objective: This study was performed to determine the long-term safety of transdermal estradiol therapy, Institution: GATA, Haydarpaşa Training Hospital, Clinic of Gynecology and Obstetrics, İstanbul. Material and Methods: This study was performed on 40 postmeopausal women to determine the long-term safety of transdermal estradiol therapy. The group I with 20 subjects received 0.05 mg transdermal estradiol for 25 days of each 28 days cycle while the group II with 20 subjects received the same dosage of transdermal estradiol plus 10 mg of medroxyprogesterone acetate, given from 1 year. Vaginal bleeding patterns and endometrial histology were characterized. While the subjects recorded bleeding patterns daily, endometrial biopsies were performed at 6th and 12th months. Results: Bleeding onset, duration and quantity were similar for both groups. The incidence of hyperplasia was 35% and 5% for groups I and II respectively. All cases of hyperplasia in group I were treated with sequantial medroxyprogesterone acetate for 3 months, followed by rebiopsy. It was noted the normal endometrium at all. Conclusion: Transdermal estradiol therapy is safe with respect to knowledge observed from endometrial histologies if it is administered with a cyclic progestin.
