ÖZET
Amaç: Yasal tahliye öncesi misoprostol kullanımının servikal açıklık ve dilatasyonun kolaylaştırılmasına etkisinin araştrılması. Bulgular: Çalışma grubunda 7.9±1.6 servikal açıklık sağlanırken kontrol grubunda bu değer 5.5±1.4 mm idi.Yasal tahliye işlemi sırasında uygulanan dilatasyon çalışma grubu olgularının %92 sinde çok kolay veya kolay gerçekleştirilirken, kontrol grubunda bu oran %26 ile sınırlı kalmıştır. Kanama miktarı çalışma grubu olgularının %68 inde (35) az ve %31.4 ünde (16) normal olurken kontrol grubunda %11.9 (5) az, %73.8 (31) normal ve %14.3 (6 ) olguda fazla olmuştur.Yan etki olarak çalışma grubunda bir olguda abortus, 4 olguda vaginal kanama ve 8 olguda bulantı görülmüştür. Sonuç: Yasal tahliye öncesi misoprostol kullanımı servikal açıklığı sağlama ve kolaylaştırmada etkili bulunmuştur. Dilatasyonun zorlu olacağı beklenen olgularda preoperatif kullanımı işlemi kolaylaştırma, hasta ve hekimi rahatlatma, işleme bağlı kanam ve diğer komplikasyonları azalt-mada yararlı olacağı kanısındayız.
ABSTRACT
Objective: Evaluation effect of misoprostol on cervical dilatation, facilitation of further dilation and bleeding in patients of first trimester pregnancy termination is the aim of this study. Materials and Methods: Ninetynine women applied for pregnancy termination, who were 6-10 weeks pregnant, were enrolled in this study.Misoprostol 400 µg was given to 52 cases a day before the procedure at 22:00 o'clock to take orally.Control group was not given medication and was consisted of 42 cases. Cervical dilation was measured before the procedure in two groups by identification of the largest Hegar dilator that could pass through the cervix without resis-tance.Easiness of the further dilation and bleeding were evaluated subjectively. Results: Cervical dilation was 7.9±1.6 mm in study whereas it was 5.5±1.4mm in control group.Further dilation was very easy in 92% of the study group and only 26% of the control . Bleeding during procedure was little in 68% and normal in 31.4% of the study group, in control group it was little in 11.9%, normal in 73.8% and heavy in 14.3%. As a side effects, in one case abortion, in 4 cases vaginal bleeding, and in 8 cases nausea were noted. Conclusion: Misoprostol was found effective in establishment of cervical dilation and facilitation of further dilation. We suggest that preoperative use, especially in cases with rigid cervix such as nulliparous, will facilitate the procedure, and decrease the discomfort of patients, complications such as bleeding and uterine perforations.
